Contacting Patients Through Chat
Staying in touch with patients is an important part of monitoring progress and maintaining RTM services. The Chatbox Feature is a tool that can be used for a variety of purposes and should be used at least once per week.
Note: Patients should never be contacted using personal phone numbers. All text based communication must be sent exclusively through the PhyxUp Health chat feature.
Patient Check-in
When patient engagement is limited, such as when little data has been uploaded or calls are not being answered, the chat feature can be an effective way to check in with patients and understand how they are doing.
The platform provides several pre-written message templates to help get started, but avoid relying on them too heavily. These templates are intended as guidance only. Be sure to personalize each message so patients feel their care is thoughtful and individualized, rather than generic.
When to Chat with Patients
As a general rule, use the chat feature when the information being shared does not require a phone call, or when a patient is unresponsive to calls and has not uploaded data for some time.
The chat feature can be used regularly for the following purposes
- Scheduling calls
- Patient check-ins
- Providing feedback on uploaded data
- Reminders to engage with assigned HEPs
